英辞郎によると、

1. 〔試合・競争などにおける〕2位の人［チーム］、次点の人
2. 〔選挙の〕次点候補（者）

なお、「2位の人」になっていますが、「2位以下の人」の場合もあります。つまり、2位、3位、4位の人もrunner-upと言うことがあります。その中で順位をつけるのであれば、first runner-up, second runner-up, third runner-upになります。
